Payment is due when services are rendered. All charges are ultimately the client's responsibility. It is the client's responsiblity to verify benefits prior to the first appointment. Insurance Benefits Checklist 1) Contact your insurance company using the 1-800 number listed on your card. Let them know you are inquiring about outpatient mental health benefits. Often, health insurance companies have a separate company for mental health benefits. 2) Determine whether the preferred therapist is an in-network or out-of-network provider for your mental health benefits. (Under some insurance companies, J. Todd Baker's name may also be listed under Jason Todd Baker or Jason Baker). For a list of insurance providers with whom we are affiliated, click here. 3) It is important to ask about the deductible, co-pay, number of visits allowed in a calendar year or lifetime of the policy AND find out if an Authorization Number is required before your first visit. Also, get the mental health claims address for your insurance. Check each of these benefits for an in-network provider and an out-of-network provider. 4) Contact our office to schedule an appointment time. You may use the "Make Appointment" link at the top of this page. Most appointments are scheduled within one week. |
